So, the main issue with this item is that it crosses into Oorigami clan territory. We've allowed some instances of additional arms, but the main ones I can think of are Mayonaka (due to spider physiology), puppeteers having additional puppet arms, but they had to actively control them like puppets, and the Hotei religion, which uses spectral hands rather than proper arms.
Aside from that, I believe they'd have to be conductive rather than reactive. Reactive means that chakra is applied, and then a specific effect occurs. Conductive means that you can move the chakra through it as if it were an extension of your body.
I am also concerned that you are attempting to increase handseal speed. The equipment has the fast reflexes SA (which it is already splitting between reflexes and dexterity), but the physical description mentions that the other hands can be used to render handseals; thus, it appears to be an attempt to slip handseal speed, which is made even odder when the pathways skill states that only two arms can use jutsu. How can you use other arms to perform handsigns as part of a jutsu if they are unable to use it?
Medical Ninjutsu Prowess is a definite no. Even if we did allow for such a broad discount (which we do, but only as a clan ability, such as Yamakira receiving a massive doton discount), how does it make sense that these arms would simply reduce the chakra cost of all medical jutsu? It's a ridiculous request.
From what I can gather, I imagine you want two sets of arms that can constantly heal, and two arms that can fight with weapons (mostly because you mentioned they're terrible in melee fights, but you also mentioned weapons can be handled just fine), two that can use ranged attacks or ranged jutsu as needed, and their original two for defense.
The way the item is currently written is either a no or requires extensive editing. The ability to use extra limbs is clan-locked and if it were done via prosthetics, then it'd be puppetry-locked as clarified by the Narashi puppet fanatics clan.
Even if we ignore the above, the most significant flaw is that the arms can perform jutsu with no downsides and that all medical techniques cost half as much.
And if you're going to edit these and somehow come up with a way to make this work, I'd recommend adding a "Coordination" SA to help with balancing. The idea is that the human brain isn't used to controlling that many arms, so it would require extensive training and physical therapy to learn how to do so correctly. Of course, this does not imply that they are all now able to use jutsus and engage in combat at the same time, as this would go too far into Oorigami clan territory.
Note: Another thing I noticed in the first SA you mention: Due to the cells the user can also heal the arms if cut off or injured. Self-repairing abilities are a separate SA of its own with a cost.
And technically as a med-sole, you should not even have an S-rank equipment, that is not medicine or poison. As a sole-non buki spec you are allowed max 1 A and 3 B-ranks.
